We have attempted to reach all patients by test, email or phone with important information and most were contacted.  However, three did not have working mobile phones or did not respond to emails, text messages, or phone calls to contact numbers on file.  If you are a patient that was unreachable, then a partial prescription will be called into the last pharmacy where you filled the prescriptions that will last until I am again available August 11, 2018.  You must complete the survey under the Scheduling Tab to have a subsequent final prescription plus one refill issued to you while you locate another physician to prescribe.  The phone contact 727 469 3040 will no longer ring through to the physician, but text messages and voice mail via that number will be received.
